Nadia Garay has two decades of experience in the healthcare industry, Nadia began her career in the medical fiend back in 2006 as a front desk receptionist. Through hard work, dedication, and a passion for exceptional patient care, she advanced into leadership roles, successfully managing multiple urgent care centers, including one recognized as among the most successful in the country .

Today, Nadia proudly serves as the Practice Manager at Middletown Medical Group, where she is committed to creating an exceptional experience for every patient who walks through the doors. She believes that outstanding healthcare begins with compassion, teamwork, and a patient-first approach. Nadia is passionate about building a welcoming environment where patients feel heard, valued, and confident in the care they receive.

 Dr. Varma is a board-certified Emergency Medicine physician with over 20 years of experience caring for patients across New Jersey's emergency departments and urgent care centers. Trained in London and completing his residency at Newark Beth Israel Medical Center, he brings both international medical breadth and deep local roots to his practice.

Whether it's a sudden illness, an injury that can't wait, or a concern you just need seen today, Dr. Varma is here for it. Having spent years on the front lines of busy emergency departments; including as Chairman of the Emergency Room at Community Medical Center, he is experienced in evaluating everything quickly and thoroughly, so you get the right care at the right time without the wait of a traditional ER.

As a former Area Medical Director for a network of 16 urgent care clinics and a current Medical Director, Dr. Varma has spent his career making quality healthcare more accessible and that same mission drives his role here. He holds an MBA alongside his medical degree, ensuring that your experience as a patient is as smooth and well-organized as it is clinically excellent.

Dr. Varma can see patients ages 1 and up and can treat a variety of illnesses. We are pleased to have him on Mondays and Wednesdays from 2-8pm.

 Nina Chacko, PA-C is a board-certified Physician Assistant providing comprehensive primary care for adolescents and adults. She focuses on preventive care, wellness visits, and helping patients manage both acute and chronic conditions so they can maintain and improve their long-term health.

Nina earned her Bachelor’s degree from Seton Hall University in 2011, followed by her Master of Science in Physician Assistant Studies from Seton Hall University in 2013. She is board certified through the National Commission on Certification of Physician Assistants (NCCPA) since 2013.

She believes in a patient-centered approach and takes the time to listen carefully to each patient’s concerns. Nina works collaboratively with her patients to develop personalized treatment plans and emphasizes education so they feel confident and informed about their health.

Nina provides care for a wide range of primary care needs, including annual physicals, sick visits, preventive screenings, and the management of chronic conditions such as high blood pressure, diabetes, and high cholesterol. Her goal is to create a welcoming environment where patients feel comfortable, respected, and supported in their care.

Dr. Yasmen Atef Srour is a board-certified Family Medicine physician, board-certified in Aesthetic Medicine, and board eligible in Longevity Medicine. She is passionate about providing compassionate, evidence-based care that helps patients improve their health, function, and overall well-being.

Dr. Srour earned her Doctor of Medicine from Ross University School of Medicine and completed her Family Medicine residency at Rutgers Robert Wood Johnson University Hospital, where she received advanced training in comprehensive outpatient and inpatient medicine, women’s health, pediatrics, geriatrics, and preventive care.  

Throughout her career, Dr. Srour has cared for patients in private practice, hospital, urgent care, and academic settings. She has experience managing acute and chronic medical conditions, preventive care, weight management, hormone therapy, and office-based procedures.  Dr. Srour has been recognized with the Top Doctor Award and she was featured in Forbes for her work during the COVID-19 pandemic.  

Fluent in both English and Arabic, Dr. Srour believes that exceptional medical care begins with listening. She is committed to building lasting relationships with her patients and empowering them to make informed decisions about their health through personalized, patient-centered care.

Dr. Michael Subik, DPM is a board-certified reconstructive foot and ankle surgeon licensed in New Jersey and New York with more than 25 years of experience treating complex foot and ankle conditions. He specializes in limb salvage, primary and revision reconstruction, foot and ankle trauma, total ankle replacement, and advanced diabetic foot care. Recognized as a leader in his field, Dr. Subik has consistently been named a New Jersey Monthly Top Doctor by his peers since 2015.

In addition to caring for patients, Dr. Subik is dedicated to advancing foot and ankle surgery through education and innovation. He is the Founder and Director of the North Jersey Reconstructive Foot and Ankle Fellowship Program and serves as Director of the Podiatric Medicine and Surgery Residency Program at St. Mary's General Hospital. Throughout his career, he has trained surgeons across the United States and internationally in advanced reconstructive techniques, external fixation, total ankle replacement, and diabetic limb salvage. He also serves as a consultant to leading medical technology companies, helping develop and refine innovative surgical techniques and devices.

A lifelong New Jersey resident, Dr. Subik was raised in Belford and lives in Monmouth County. He is proud to bring his expertise to Middletown Medical Group, where he combines state-of-the-art surgical care with a compassionate, patient-centered approach. His goal is to help every patient restore mobility, relieve pain, and return to the activities they enjoy through individualized treatment plans using the latest advances in foot and ankle care.

Sweta Ramnani, PA-C is a board-certified Physician Assistant dedicated to providing compassionate, evidence-based care for her patients. She has extensive experience evaluating and treating a wide range of acute medical conditions and is committed to helping patients feel informed, supported, and confident in their healthcare decisions.

Sweta earned her Master of Science in Physician Assistant Studies from Wagner College in 2019. She began her career in the Intensive Care Unit at Mount Sinai, where she cared for critically ill patients during the height of the COVID-19 pandemic. She then transitioned to urgent care, where she has spent the past five years diagnosing and treating acute illnesses and injuries while providing accessible, high-quality care to her community. She currently continues to practice in urgent care.

Sweta believes in building strong relationships with her patients through open communication, empathy, and patient education. She strives to create a welcoming environment where every patient feels heard, respected, and actively involved in their care. She is fluent in both English and Hindi, allowing her to provide personalized care to a diverse patient population.

Julia Appleman, PA-C is a Board-Certified Physician Assistant with over 10 years of experience in Emergency Medicine and Urgent Care. She earned her Bachelor of Science in Biology from Monmouth University in 2012 and her Master of Science in Physician Assistant Studies from Seton Hall University in 2015.
Throughout her career, Julia has cared for both pediatric and adult patients, treating a wide range of acute and chronic medical conditions. Her extensive clinical experience has provided her with the expertise to deliver high-quality, evidence-based medical care in fast-paced healthcare environments.
Julia is committed to treating every patient with compassion, empathy, and respect. She believes that building trust and fostering open communication are essential components of excellent healthcare. Her goal is to ensure that patients feel comfortable, understood, and confident in their care while receiving personalized treatment tailored to their individual needs.

Erin has over a decade of experience as a Medical Assistant, she is dedicated to providing compassionate, efficient, and high-quality care to every patient she serves. She takes pride in creating a positive patient experience and is committed to ensuring each visit is as comfortable and seamless as possible. Known for her attention to detail, strong work ethic, and friendly demeanor, Erin is an invaluable member of the Middletown Medical Group team.

Outside of the office, Erin enjoys staying active and challenging herself by running marathons around the world, reflecting the same determination and perseverance she brings to patient care every day.

Yesenia has eight years of experience as a Medical Assistant, she brings a warm, compassionate presence and a genuine passion for patient care to Middletown Medical Group. She is known for her attention to detail, positive attitude, and commitment to ensuring every patient feels comfortable, supported, and well cared for throughout their visit.

Dedicated to continuous growth in the healthcare field, Yesenia is currently pursuing a degree in Diagnostic Medical Sonography. Her commitment to advancing her education reflects her passion for expanding her skills and continuing to make a meaningful impact in the lives of the patients she serves.
